Visits per day One suits most healthy adult cats. Two is better for kittens, seniors, and twice-daily medication — we'll say if we disagree.

Medication or special needs What, how much, how often, and how they take it. Say if they're difficult about it — it helps to know before the first visit rather than during it.

Your vet Practice name and phone, if you have it handy.

What makes your cat happy? The food they'll actually eat, where they hide when the door opens, the toy that works when nothing else does. This is the most useful box on the form.
